Revision Rhinoplasty, also referred to as secondary rhinoplasty, is a corrective aesthetic or functional nasal surgery procedure which is performed after a previous rhinoplasty or revision rhinoplasty, nose surgery.  There are multiple and distinct reasons for a Revision Rhinoplasty.  Every nose will heal after a Rhinoplasty.  About 5 – 10% of the time the healing may create minor issues, which may be improved by a secondary “touch-up” minor revision Rhinoplasty procedure.  But the most common causes of patients seeking revision Rhinoplasty surgery are not due to healing.  There are three serious causes for cosmetic surgery patients undergoing more nasal reshaping surgery after their previous nose job surgery:

  1. Gross surgeon error due to lack of understanding of the nose and lack of specialization in aesthetic &/or functional nasal surgery.  These patients may have had 1 or 2 or even 8 very poorly performed previous nose jobs and they require the proper revision Rhinoplasty to restore balance to their faces and lives.
  2. Inadequate preoperative dialogue between plastic surgeon and cosmetic patient resulting in variance in goals and outcome.  Essentially the nose may be a nice nose after the nose job surgery but not what the patient wanted or asked for.
  3. Patients with BDD (body dysmorphic disorder) or simply lack of understanding of nasal analysis and healing who are almost never content or happy with any Rhinoplasty surgical results no matter how good the results.  These patients often seek “perfection” in their own eyes and will continue to seek revision after revision after revision.

Revision Rhinoplasty may improve both aesthetic and functional problems. Good candidates for revision rhinoplasty include healthy patients over the age of sixteen who have realistic goals for improvement.   Poorly performed or poorly healed previous nasal reshaping surgery can create cosmetic &/or breathing problems which may need to be addressed with Revision Rhinoplasty.
Dr. Naderi is a Rhinoplasty and Revision Rhinoplasty specialist. These two surgeries make up well over 90% of his practice.  He performs revision rhinoplasty at one of the INOVA system hospitals in Northern Virginia, which include Fair Oaks Hospital, Loudoun Hospital, and Fairfax Hospital.  He also has surgical privileges as Suburban Hospital in Bethesda, Maryland, part of the Johns Hopkins Hospital system. The time range to perform a “revision nose job” by Dr. Naderi is anywhere from 15 minutes,up to 3 hours or even more to complete and is usually performed with general anesthesia although certain minor revision Rhinoplasty procedures may be done under purely local anesthesia.

About 30% of revision rhinoplasty cases are endonasal or closed, in which incisions are confined to the inside of the nose, also known as “scar-less nose job.” The other 70% are external or open, which involves incisions made on the inside of the nose as well as a very small and hidden incision made on the skin between the nostrils on the base of the nose.  Once the incisions are made, the desired shape may be achieved by repositioning cartilage or replacing missing structures. The incisions are then closed and a splint is applied to the nose.   Revision Rhinoplasty is often more complicated than primary Rhinoplasty due to scar tissue, missing tissue, malposition tissue, alterations in blood flow and skin resilience, etc.  This is why some of the top board certified plastic surgeons in the greater Washington DC area refer all of their Revision Rhinoplasty cases to Dr. Naderi due to his specialization in the field of nasal surgery. 

Bruising and swelling may be more extensive after revision rhinoplasty than after primary or original procedures. Patients generally experience very little pain though since Dr. Naderi uses pristine and meticulous surgical technique and does not routinely pack the nose. Most of the bruising fades within ten days while the swelling may take several months to dissipate. External stitches are removed in three to seven days. Most patients can return to work within a week, though more strenuous activities will need to be postponed for two to three weeks.
